 AS WE GROW AND RAISE MORE MONEY FOR THIS PROJECT WE WILL BE ADDING A SPECIALTY CANDLE, A GIFT CARD, AND EVENTUALLY HELP AIDE IN FUNERAL COSTS.

HOW TO SIGN UP/NOMINATE A LISSENCEPHALY FAMILY:

-Your name and email if it differs from nominee

-Name of Lissencephaly Individual who has passed away

-Birth & Angel date (if you have it)

-Name of Parent receiving package

-Mailing address of family receiving package

-Photo of the Lissencephaly individual
